Release of Liability
I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and / or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Charlotte Marn School of Dance, its officers, agents, employees, instructors and all affiliated entities (hereinafter collectively referred to as “Charlotte Marn School of Dance”). I hereby agree to release Charlotte Marn School of Dance and hold Charlotte Marn School of Dance harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance activities. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this voluntarily and of my own free will in exchange for the privilege of participation.
Photography & Videography
I also give Charlotte Marn School of Dance permission to photograph and/or video record my child / myself in dance activities for use in brochures, websites, and any other advertising media. I understand that said photographs and video will be copyrighted in Charlotte Marn School of Dance’s name and that I have no rights to the same and will not be compensated for the same. In order to respect the privacy of other students and intellectual property, I will not photograph nor record video on any device without the expressed permission of Charlotte Marn School of Dance.
Use of Personal Data
I hereby give clear and unambiguous consent to Charlotte Marn School of Dance and its agents to contact me via SMS, telephone call, or email regarding any class updates, programmes, events, and materials related to dance and the performing arts.
Payment Information
I understand that the obligation to pay all fees described in the brochure / application is unconditional. No fees, paid or outstanding, is refunded in whole or in part in the event of any absence, withdrawal, or dismissal of the student. No replacement class or refund will be made for any class missed. The full course fee is forfeited if no notice is given of non-attendance. I also agree to pay all incidental expenses as may be incurred by the student which include, but are not limited to, costumes, dancewear, footwear and similar items.
Terms and Conditions
Any changes to course registration(s) after payment is subject to the sole discretion of Charlotte Marn School of Dance and to an administrative charge of S$50.00. Charlotte Marn School of Dance reserves the right to amend the schedule without prior notice.
